Privacy Policy
How we handle your personal and health information.
Who we are
Recoil Physio (“we”, “us”) runs a physiotherapy, osteopathy and sports massage clinic at 212 Smith Street, Collingwood VIC 3066. This policy explains how we collect, use, store and share your personal information, including health information, under the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records Act 2001 (Vic).
What we collect
Your name, date of birth, contact details, health fund and Medicare details, emergency contact, and the health information you give us or that we record during treatment: history, assessment findings, treatment notes, exercise plans and letters to or from other health professionals. On this website we collect basic analytics about how the site is used, and anything you send us by email.
How we collect it
Mostly from you: when you book, fill in your intake form, or talk to us during a session. Sometimes from others with your consent, such as your GP, surgeon, insurer or WorkCover agent.
Why we use it
To provide and plan your care, to keep accurate clinical records, to process payments and health fund claims, to send appointment reminders, to meet our legal and professional obligations, and to improve our services. We do not sell your information and we do not use your health information for marketing.
Who we share it with
Only with your consent or where the law allows: your referring doctor, other health professionals involved in your care, health funds, Medicare, WorkCover or TAC for claims you make, and service providers who host our booking and records software under confidentiality agreements. Some providers may store data outside Australia; we only choose providers with privacy protections comparable to the Australian Privacy Principles.
How we keep it safe
Clinical records are kept in secure practice software with access limited to our clinicians and front desk staff. We keep records for at least 7 years after your last visit, or until a child patient turns 25, as required in Victoria, then destroy them securely.
Your rights
You can ask to see or correct the information we hold about you at any time. We will respond within 30 days. You can also choose to stay anonymous or use a pseudonym for general enquiries, though we cannot treat you without identifying you.
Questions and complaints
Talk to us at the front desk or through the contact details on this site. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner (oaic.gov.au) or the Health Complaints Commissioner (hcc.vic.gov.au).
Template note
This policy is sample content for a demo clinic. Have it reviewed by a professional and adapted to your practice, country and software before you publish your site.
